Harshita Brella went to Northamptonshire Police in Aug last year to report allegations of domestic abuse by her then husband Pankaj Lamba. He was arrested, released on bail and issued with an order intended to protect Ms Brella. Some two months later, she was found dead.

A Sergeant and Chief Inspector have a case of misconduct to answer “over their supervision and review of the domestic abuse investigation and associated risk assessment for Ms Brella.”
