I know I am insufferably pedantic, but there are certain things I really hate seeing in medical records. "Anti-sickness" - these are antiemetics. Use that word. "IFM" - these are inflammatory markers, but why not just write CRP because that is what it's usually a reference to.

In paediatric chest trauma needing a drain, APLS state insert the largest drain to fit past the ribs, others advocate the rule of 4.. 4× [age/4 + 4]. For a five year old this means a 24Fr chest drain... Why so big? @EddCarlton @PERUKItweep @DFTBubbles @Damian_Roland

@therunningpara Yeah 115 is low, warrants admission and management to find the cause and slowly bring it up. Infection in itself doesn’t cause it unless vomiting, diuretics and PPIs etc. sounds quite acute drop if only discharged 10/7 before though!

@therunningpara How low? Hyponatraemia doesn’t cause SOB as far as I’m aware. Lethargy and confusion yeah if very low…. Common in elderly often due to their medications 💊

There are many true advanced paramedics but also many paramedics with MSc level studies using the title with little autonomy or lacking the 4 pillars of AP. Such a variety of paramedics in so many settings and roles is exciting but hard to define limits… 2/2

We still have a variety of paramedics that work at very different levels that I’m not sure how will be addressed. The risk averse, still have ‘ambulance driver’ types and the highly intelligent with fantastic 1/x

Today, I had the opportunity to visit Edmonton Station, which will be my complex station. I was fortunate to observe a Team-base work meeting in person, and the positive atmosphere was genuinely inspiring. Meeting @AlexEwings was a highlight, as Alex kindly explained his role to me within LAS. As an NQP soon the prospect is daunting, but after this experience, I feel much more optimistic and enthusiastic about my role and look forward to supporting and serving the residents of the area but also looking forward to Team-Base working and being supported as an NQP 🚑🙌🏾
