Night crew transpo failure to thrive pt to ED. Today enrolled in our @RenCtyParamedic #CommunityParamedic in the hospital, transpo the pt home, setup for the weekend (chopped wood) and arranged to go back with a social worker Monday. #paramedic initiated and managed. #Teamwork
