Piper Alpha is an excellent case study in industrial safety. A current concern though is that industrial safety is starting to adopt "HR speak" type policies that focus far more on mental health than on the actual physical safety part. One of the core ideas is called "Human and Organizational Performance" (HOP). What makes HOP dangerous is in it's first two tenets. 1) Mistakes happen and we need to be willing to accept some level of failure from humans. 2) Blaming someone doesn't solve the problem, as everyone is only human and prone to making mistakes. So basically, HOP has decided that we should accept failure while also removing accountability. When you look at what happened with Piper Alpha, the negative consequences of HOP become immediately apparent. Every one of the events leading up to Piper Alpha was what wouod be considered a "minor failure". And yet, a lot of people died because of them. When you work in industries like oil and gas, there are hazards in this environment that will kill you in short order. The amount of damage that can be done is immense. And as such, safety controls should be rigid. Our standard should be no failure whatsoever. We should not accept it. Should failure arise, we should stop, address it, amd make sure everything is good to go before proceeding. If that means we have to blame someone to correct the problem, then so be it. The system we're building at Furean will mitigate a large number of these hazards. It's not an engineering control, but it does serve as an advanced early warning system that can give operators unprecedented insight into critical operations.

A major oil & gas disaster like the Piper Alpha disaster didn’t start as a catastrophe: it started as a small issue that escalated. That’s how most accidents happen. Not wearing PPE might feel harmless in the moment…until it isn’t. In this industry, safety is not luck.
