When Forgetfulness is Not Just Stress As a neurologist, I often see young adults who come to me worried about memory lapses or irritability. Many times, these are brushed aside as stress-related, especially in today’s fast-paced world. But sometimes, the story is different. One such patient was a 33-year-old IT professional who came with a 3–4 year history of forgetfulness and irritability. He had no major medical problems, but these subtle changes were affecting both his work and family life. His relatives felt he was simply “stressed out.” On further questioning, I learned that he was a strict vegetarian. That detail raised my suspicion. I ordered a set of tests, including an active Vitamin B12 (holotranscobalamin) level. The result was striking- just 10 pmol/L (normal >70). This was severe Vitamin B12 deficiency. Why it mattered Vitamin B12 is crucial for brain and nerve function. Deficiency can present with neuropsychiatric symptoms-forgetfulness, irritability, mood changes, depression, or poor concentration, sometimes even without anemia or obvious nerve damage. In his case, years of vegetarian diet without supplementation had led to a silent depletion of this essential nutrient. Management and Outcome He was started on intramuscular Vitamin B12 injections, followed by a maintenance regimen. Alongside, I advised him to include dairy products and fortified foods in his diet. Over the next few weeks, there was a noticeable improvement in his mood. Over months, his memory and concentration improved significantly, and his family remarked that “he was back to his usual self.” What can we Learn from this? This case is a reminder that Vitamin B12 deficiency is extremely common in India, especially among vegetarians, and often goes unrecognized. Because its symptoms overlap with “stress” or psychiatric issues, many patients are misdiagnosed or remain untreated for years. The good news: a simple blood test and timely treatment can completely reverse the condition if caught early. ✅Take-Home Messages For the General Public 1. Persistent forgetfulness, irritability, or fatiguemay not just be stress. Seek medical advice. 2. Vegetarians and vegans are at higher risk of B12 deficiency. Fortified foods or supplements are often necessary. 3. Early treatment prevents irreversible brain and nerve damage. For Junior Doctors 1. Always consider Vitamin B12 deficiency in patients with neuropsychiatric symptoms, even if CBC is normal. 2. Remember: Absence of anemia does not rule out deficiency. 3. Active B12 (HoloTC), serum B12, or homocysteine/MMA can help confirm diagnosis. 4. Prompt supplementation (parenteral in severe cases) is safe, inexpensive, and often life-changing.
