“Doctor… it’s the worst headache of my life.” She walked into Medicine casualty holding her head, not crying, not shouting, just looking scared and exhausted in a way that makes your gut tighten. Her husband was the one speaking fast. “Sir she didn’t want to come. Kids’ exams are going on… she kept saying she’ll manage.” He paused and added, quietly: “But she never looks like this. Never.” So he didn’t wait. He brought her anyway. She told me the pain started suddenly. Like a switch flipped inside her skull. Within seconds it became unbearable. Worst headache of her life. Vomiting. Light hurting her eyes. And something in her face said this wasn’t “just a headache”. In the ER, we see hundreds of headaches. Most are harmless. But this one had the words we never ignore: Thunderclap. Worst-ever. Sudden onset. So we rushed her for CT. The scan came back. "Subarachnoid hemorrhage" Blood where blood shouldn’t be. One of those diagnoses where the patient may still be talking today… and not get the chance tomorrow. Her husband sat there frozen. And then he asked the only thing families ask in that moment: “Doctor… will she be okay?” I looked at him and said: “You brought her at the right time. That’s the difference between treatment… and tragedy.” We moved fast. BP control, monitoring, pain control, seizure precautions. Neurosurgery was informed immediately and she was shifted to the Neuro ICU without delay. Because in SAH… minutes matter. Sometimes saving a life isn’t about medicines. It’s about one person noticing: “This isn’t normal.” And acting fast.
