She was my very first colorectal cancer patient 5 years ago, who received dual immunotherapy despite her tumor not primarily classifying for immunotherapy.
Her cancer had spread to the lungs without any options to perform surgery. She had gone through various cycles of chemo with no meaningful effect in the end.
But her tumor showed a high reading of another marker called TMB - tumor mutational burden.
✅ After 3 cycles of immunotherapy all the lung metastases were gone and she remains with no evidence of disease until today. A tremendous success story of modern personalized oncology.

The talks in Islamabad didn't fail because Iran wasn't serious. They failed because the wrong person was in the room.
Ghalibaf led Iran's delegation through the Islamabad talks and carried a 10-point framework.
He also doesn't control Iran's enrichment infrastructure, the Strait of Hormuz, or the military apparatus that holds everything Washington wants surrendered. The IRGC does.
Hours after the talks collapsed, IRGC commander Ahmad Vahidi publicly accused Ghalibaf of betrayal for shaking hands with the American VP.
That wasn't a private disagreement. It was a message to both Tehran and Washington.
The IRGC banned missile program discussions before talks even began. They embedded vetoes inside the delegation.
They operate under a decentralized command doctrine designed to survive regime decapitation, which it did after Khamenei was killed.
That same doctrine makes binding diplomatic commitments structurally impossible.
Vance couldn't get an affirmative commitment that Iran won't seek a nuclear weapon. But Ghalibaf was never in a position to give one.
The institution that holds what America wants wasn't in the room.

@ImtiazMadmood If American and Israel continue for another 1 month, Iran wll b gone for good. They should not stop, plan B , let Saudi, uae, Kuwait collect trillion dollar and give it to Israel. Leave everything to Israel 🇮🇱. This should be enough for 200k foreign nationals ground troops

It seems that very little remains of Iran's ayatollah regime, and a radical political change in that country would be imminent.
The USA and Israel destroyed its navy and air force, and the Revolutionary Guard is decimated and in disarray.
It's running out of missiles and drones, its main weapons, and it has lost control of the Strait of Hormuz.
To top it all off, the people reject it after a crackdown that left 30,000 dead and it attacked several of its Arab neighboring countries, which now oppose it.
The United States is beginning its commercial strangulation and international isolation in such a way that political change in Iran is inevitable.

My thoughts on the US/Iran negotiations:
Vice President Vance says the U.S. has issued a final offer, signaling Washington is not planning to reopen terms.
Tehran is likely to push for continued talks, leveraging its track record as a super negotiator, everyone knows the Persians know how to negotiate, just try buying a Persian carpet.
The current U.S. position goes well beyond the Joint Comprehensive Plan of Action, expanding from nuclear limits to 4 core demands:
1. Nuclear program restrictions
2. Ballistic missile limits
3. Halt funding to regional proxies
4. Keep the Strait of Hormuz open
These are far broader and more demanding than previous frameworks.
President Trump is also signaling escalation: if Hormuz access is restricted, the U.S. could move toward a full naval blockade of Iran’s oil exports, aiming to cut off regime revenue.
Let’s see where this goes.
