#ACC26 kicking off with Wild @WApplefeld kicking off our inaugural critical care cardiology pre conference - packed house with > 200 attendees who came in early for this effort - looking forward to this day! MCS, vent trouble shooting, and reconciling where the guidelines meet real practice in the modern CICU.

Appreciate the data and thoughtful analysis. I emerged with some slightly different takeaways; mainly, that fellow seeking a training program are largely rational actors who prioritize volume, complexity, and acuity, and skew towards centers that are known to offer these most consistently. I share your concern that we have significant unmet needs in treating heart failure in our underserved and rural communities, but do not think we will meet this challenge by pushing our trainees away from our high volume centers, but rather continuing to endeavor to empower our primary care and general cardiology colleagues who care for the bulk of heart failure in this country. Also, special shout out to Dr. Miriam Jacob, who has continued to fill our fellowship with such incredible talent in these trying times with the support of @NutritiHF and @rcstarling . Hope to see some of you at our alumni event tonight; having the largest fellowship has also led to an incredibly robust post graduation network for career advancement and networking and it should be fun tonight! @tavrkapadia @RanLeeMD @vbluml @iyadisseh

Here are all AHFTC fellowship programs that offered spots in last 5 years, and # of fellows they matched. 13 programs trained 50% of fellows. #HFSA2025 Skewed uneven distribution. Mega centers train most of the field. Insufficient exposure to community and rural needs. Worrisome.
