André d’Avila demonstrates substrate-based VT ablation guided by sinus rhythm and coronary sinus pacing, without the need for VT induction. The target site is identified on substrate mapping, showing a shared channel responsible for two ventricular tachycardias. Ablation at this single location allows both VTs to be eliminated through electrical scar homogenisation. Only around 15% of the low-voltage area is treated, achieving an effective lesion set while preserving surrounding myocardium. The patient remains free from ICD therapy following the procedure. This case highlights how targeted substrate ablation can simplify complex VT management and reduce reliance on VT induction. #EPeeps
