You could also speak to the GP surgeries to ask them to change how their patient data is set up, to improve what’s visible via SCR-AI.

Great point by our SW England Lead: Frequent Callers are vulnerable & complex with needs that are not being met with various reasons. Care needs to be individualised and person-centred. The SCR+AI will allow you to see what’s been done to date. Consider what support is needed.
