Can we reimagine chronic disease and cancer research through an engineering lens—one that seeks root causes, not just symptom management? Historically, NIH- and pharma-funded randomized controlled trials helped us conquer acute diseases—but they fall short in solving complex, chronic conditions. We need a new systems approach. With AI and big data, we now have the tools to look at patients as whole systems. Imagine tackling the top 20 chronic diseases like engineering problems: AI gathers multi-layered patient data (genomics, -omics, biomarkers, lifestyle / gene expression, family history). Then researchers, clinicians, and engineers use “5 Whys” and root-cause analysis to trace dysfunctions to the molecular level. Patients are no longer passive. They become co-creators of solutions—working with expert teams to design ethical, human-centered translational research. The NIH Whole Person Health Coalition, launched in December 2024, is a step in this direction. Let’s build on it. With collaboration and innovation, we can move from managing disease to reversing it.
