@doctimcook @SafeAirway For this reason, trainees and all of us must continue to practice with DL. In your own words, you need at least 50 tubes with any device. So all of us need at least 50 DL’s! VL therefore can’t become universal.

Yesterday I posted a slide on the benefits of VL That was unfair so I've made one for DL too so they can be compared Have I missed anything?

@doctimcook @SafeAirway Interesting debate last week. ED and retrieval physicians have had VL embedded for years. Just be cautious of the unintended effects, we’ve seen decline in good technique. So when VL is unavailable/fails, potentially higher rates of failure.

Paraphrasing @RuthParsell - video laryngoscopes are prone to device failure, particularly in the prehospital settings. Have they also eroded the discipline of stepwise airway instrumentation that could optimise our view of the chords? @SafeAirway #SAS2023

@EastCoast_Para Agree. The Warwick data re: numbers actually treated vs incidence looks good. Do they have tighter criteria about when to initiate resus? If so, can u share?

@MallinsonT Our study suggested that the re-tension rates were unacceptable if no drain inserted. Our general approach is finger only when in TCA but, if ROSC, or tension physiology only, drain. We also use the Frontline drain kit and find it rapid to insert and excellent.

@annecreaton If you are open for business people rightly expect attention. Doctors should place the ED on bypass otherwise.

Terrible patient, family and staff. EDs need staff& space to assess undifferentiated patients, meaning admitted pts must go to wards. Answer isn’t bigger EDs. Coroner finds Sunshine Hospital emergency department delays contributed to man's death - ABC News abc.net.au/news/2022-10-0…

@DrJohnOrchard In parallel, ED/hospital mgt actively encouraged these presentations by setting up Fast Track models of care. Many of these models are the only thing propping up KPI’s in treatment times as admitted pt’s languish in bed block. Our own worst enemy sometimes.

Emergency Departments in Australia are starting to get completely overwhelmed in every state. I don’t have all the answers on how to fix this, but...one measure that could relieve some pressure if Medicare is prepared to reverse 3 decades of bad policy….1/🧵

@cliffreid More than ever this term needs to make a comeback! Better understanding of the pt groups it matters in… non compressible haemorrhage etc. and where speed to/fro important. Dangerous to throw the baby out with the bath water 💧
