@ballzy_physio @DonMoxy MSK and orthopedic are different in the UK but are similar in the USA. Most US books use the term orthopaedic physiotherapy that we call MSK in the UK. In Nigeria we use both terms almost like the US: Ortho inpatient/Outpatient

@Ev256 @ibukun_tayo @doctrLakes Uncle, it doesn't always work that way. In the UK for example, without the NHS most people won't be able to afford healthcare. The USA is another example where people lament on healthcare.

My mum died at Ekiti State Teaching Hospital, and the pain of how it happened still haunts me. About two hours before she passed, she was rushed into the emergency room, desperately needing help. Yet there was no doctor available to attend to her. As this was unfolding, my sister called me in distress. I immediately drove from the Federal Teaching Hospital, Ido, all the way to Ado, praying I would arrive in time. When I got there, my heart sank. My mum was lying on the hospital bed, restless, weak, and clearly fighting for her life. I kept asking what was wrong, begging for answers, only to be told there was no doctor to see her. I confronted the nurse on duty, and her response shattered me. She said the emergency unit had only one doctor, and he was “busy.” Busy, while my mother was dying in front of us. With no help coming, I had to step in and set up an IV line for my own mum, something no child should ever have to do. Moments later, she suddenly became unresponsive. Just like that, she was gone. That was how I lost my mother,not because her condition was untreatable, but because help never came when it mattered most. Nigeria’s healthcare system is broken. Until it is completely overhauled, innocent lives will continue to be lost unnecessarily, and families will keep carrying pain that could have been prevented.

Stroke rehabilitation requires a licensed physiotherapists and evidence-based care. Sadly, in Nigeria, regulation is weak and anything goes. This patient will later come down with a lot of complications and now come to the clinic expecting magic to happen.

My son was 16 when he was hit by a drunk driver. He was in a coma for 3 months. The neurologist sat us down in a sterile conference room and laid out the scans. 'His brain stem is intact,' he said gently. 'But the rest... it’s dark. If he wakes up, he will be a vegetable. He will never speak, never know you, never feed himself. My 2025 wrapped. 1. Got closer to God. 2. Travelled and got paid to speak accross 17 cities globally. 3. Signed and became an official partner of MTN the biggest telecommunications company in Africa. 4. Took 5 children from Nigeria to America and won multiple chess medals—from Atlanta Georgia, to New York City including a gold medal at the United Nations games. 5. Spoke at the Harvard Africa Business conference, Graduate school of design, MIT and New York State university. 6. Delivered a Keynote speech at the United Nations. 7. Our kids won gold at the key academy chess masters tournament and tied for first place at the Ecobank chess tournament against 500 top schools in Nigeria. 8. Set a new world record for the longest chess marathon in recorded human history alongside my brother Shawn Martinez for 64 HOURS in the heart of Times Square. 9. Forever grateful for my friendship with Russ, our work @thegiftofchess and its continued expansion accross Africa and globally. 10. Got into the US state department IVLP fellowship alongside 20 fellows from around the globe. 11. The chess in slums innovation Hub with Lufthansa airlines Help Alliance came fully alive this year and we did high level tech training for thousands of children from Design, robotics,chess to AI. This got us 2nd place at the STEMite NAFESS innovation challenge. 16. Visited the Duolingo Headquarters in New York with 5 children from the chess in slums project, spoke to the entire product team and helped Beta test the new chess course. 17. Earned the most and gave the most I ever have. 18. Partnered with my friends @hackSultan and @SamuelOtigba to give out free Laptops weekly to young undergraduates with internship opportunities. 19. Launched my first Children’s book written by the legendary @lolashoneyin . It went on to win the children’s book of the year at the Akada book festival. 20. And my most important achievement this year is we sponsored the education of thousands of children, gave out many scholarships and have kickstarted our 10 year vision to build the biggest chess academy/free school in the world. I gained so much this year but it was also filled with many painful losses and hard lessons. My health also suffered greatly as I completely neglected self. I hope to be kinder to my body next year and spend more time with my loved ones. Above all, I’m grateful. Thank you all for being a part of my unfolding story and I hope you have an incredible 2026. I still believe in what my eyes do not yet see. Keep going dreamer❤️

@joshibza This is not true. We have a lot of Private Physiotherapy Clinics who do not require Physician referrals. The post did not exclude the doctor management rather it emphasizes consulting Physiotherapy. If all low back pain must go through all doctors it shows how less busy you are!

I took my time to listen, and I will repeat it again and again: our system operates on physiotherapy referrals. All the conditions he mentioned require medical assessment, and it is bad practice to exclude the doctor. Our system does not operate on direct access to physiotherapy, especially in our care setting. Saying otherwise is misleading. Many conditions that are normally referred to physiotherapists still require a doctor’s involvement. Mentioning conditions such as chronic back pain, arthritis, and contractures etc... He said "A lot of things on a normal it should be physiotherapists......blablabla

Listen to his message again…but this time, slowly. He never said a Physio should manage alone, neither did he say a doctor has no business with them. He’s mesage is for the doctors to normalize referring to physiotherapists. PS Patient management is a team work not one man job

@joshibza Ruling out red flags is legally supported in Nigeria for Physiotherapist. Moreso, no physiotherapist should manage red flags independently. It's a red flag and so it requires immediate referral to the appropriate team.

While first-contact physiotherapy is recognized in countries like the UK, Australia, and the US, where physiotherapists can independently assess, manage, and refer patients, this model is not fully established in Nigeria. In practice, physiotherapy care in Nigeria usually depends on physician referral, and the regulatory framework does not formally authorize autonomous first-contact practice. Although patients sometimes self-present, the system, hospital policies, and professional regulations still emphasize doctor-led pathways. Nigerian physiotherapists may assess and provide care, but comprehensive first-contact practice, including ruling out red flags and managing serious pathology independently, is not legally or structurally supported nationwide. So, if you dey 9ja behave like person way dey 9ja.

If a patient presents first to a physiotherapist with back pain, the physiotherapist will carry out a holistic assessment to identify and rule out red flags, and refer appropriately when serious pathology is suspected and indicated. Go and research about first contact physios

It's not shallow! The first thing isn't to get a diagnosis. In some cases, it takes longer and the patient suffers. The priority is to rule out red flags especially when it comes to spinal cases. It's important to understand the guidelines you work with

@Mario_Sneh That's quite shallow. You are oversimplifying the issue. First thing first, a diagnosis needs to be made. Having back pain, doesn't automatically make it a non-Dr. issue. A doctor would take history, examine, and possibly investigate, then send you to a PT if indicated.

@Bilebargio @Odoctor_ng @Mario_Sneh Low back pain assessment comes with gold standard red flags which includes, Ca, infection, CES, trauma etc. Any healthcare professionals seeing this patient is supposed to rule out these flags and refer as appropriate. This is Making Every Contact Count (MECC).

@Odoctor_ng @Mario_Sneh So if a patient is having lower back pain From metastatic prostrate cancer, he will first go to the physiotherapist and then the physiotherapist will now make diagnosis and refer to see the physician.. this kind of thinking is a problem.

@Ademola_Alesh @Mr__P1 Apt. That frozen shoulder as a differential can also come out as Shoulder OA, calcific tendinitis, Rotator cuff, Shoulder impingement after investigation. Shocker is that it can even come out as Cancer. People underestimate the accuracy of investigation.

I think you missed the point. Let me give you the simplest example of a clinical diagnosis of Frozen shoulder. Using patients demographic, MOI, Hx of DM, capsular pattern restriction in ROM, clinical objective tests, etc, a Dr or a Msk Physio can easily diagnose Frozen Shoulder. However this is a working diagnosis! with many differentials. so still a probability. You still need a diagnostic test to confirm. It’s just that in practice, we don’t need to because we are trained to follow our clinical instincts and provide treatments. So in most cases, to “confirm” a diagnosis, you need a diagnostic test.
