Thanks Nigel, even better if we usually use population health segmentation to target SMR to key cohorts ie frail elderly, in community settings AND records joined up so all can see and edit. Good stuff and glad you are doing well.

In older adults 5-10% of hospital admissions are related to adverse drug reactions. Medication reviews are an important aspect of acute care and could prevent future admissions. Also important to communicate clearly any changes with primary care colleagues
