Yes Very real. I had really nasty symphysis pubis dysfunction with my 2nd and 3rd pregnancies and was on double crutches from month 5 till birth of both . Was on physiotherapy, hydrotherapy, acupuncture and Dehydra codeine throughout the pregnancy. Pain was awful and continued for months after birth through CS. So it's no joke..But not all women will have that but those who do struggle a lot
