Before you get all exercised over how much better healthcare workers wage prospects are in the private sector in New Zealand, remember that GPs have had a strict fees review process for the last 15 yrs resulting in effective wage freezes for practice owners over this time.

Report Focus was on rearranging the deck chairs on the titanic. That’s what’s great about the announced changes - The aspiration to do better at a community level

What would it cost to replace this: “Our assessment showed they don’t meet the service criteria.” with this: “We had a good conversation, talked things over, & decided that this service isn’t offering the right kind of help, but we thought together about some alternatives."

Hate to dampen people's enthusiasm for the 'NZ NHS' etc, but the key feature of the NHS is *not* its centralisation but its free-at-the-point-of-delivery healthcare. Which is not part of this announcement.

@HealthtekNZ From this it appears that “Localities” will be the new PHOs, possibly stripped of their governance role (which will come from higher up)? May not make so much difference to already locality based PHOs such as @WellSouth1 but cities with multiple PHOs will see the biggest change.

@docdanz007 @WellSouth1 PHOs in general have struggled for a role because of indirect funding (via DHB) and an over emphasis on compliance. Maybe localities will bring a more holistic view to health so that funding is tied to patient and not to a specific disease

This worries me, in the absence of detail. Centralising the strategy, funding, procurement, IT, etc makes sense but over-centralisation of front end delivery of programmes goes against the (admittedly dead-in-the-water) Primary Healthcare Strategy of 15 yrs ago. #NZHealthReforms

This is one of, if not the, biggest shake-ups to New Zealand’s health sector in history. All DHBs will be replaced by one national agency - Health NZ. The Govt will create a new Māori Health Authority and there will be more focus on preventing negative health outcomes.
