The study behind this is a April 2025 PNAS paper (Yoon et al.) using 81 monozygotic twin pairs discordant for MS. Fecal analysis found 51 differing taxa (mostly Firmicutes enriched in MS twins). They then took ileal samples from 4 pairs and transplanted into germ-free transgenic mice prone to MS-like EAE. MS-derived microbiota triggered EAE in ~60-65% of mice (vs. ~10-20% for healthy twins), with gut inflammation, Th17 shifts, and demyelination. Two low-abundance Lachnospiraceae—Eisenbergiella tayi and Lachnoclostridium—bloomed in sick mice and are implicated as triggers. Certainty: Solid proof-of-concept for a functional role in susceptible models (stronger than correlation alone), but limited by small transplant cohort (n=4 human pairs, ~37 mice) and mouse-human differences.
