@MilitaryHealth @DeptofDefense The underlying assumptions of BRAC were wrong. Building a combined Walter Reed and Bethesda on top of an aging Bethesda was a mistake and the DOD, the government and the tax payers are continuing to pay for those decisions.

@VandaTorousMD I personally don’t like atypical unless I am worried about a malignancy and can’t quite call it or exclude it. As such in your case I would call benign and give differential since I don’t see anything concerning for malignancy. My 2 cents.

@dmlarsonpath1 'Benign' is not a Milan category, non-neoplastic is. Neoplasm is still on the differential in this case. I clarify when I use the category atypical that it may represent a neoplasm or non-neoplastic process& in cases w amylase crystalloids the particular neoplasm of concern is WT

The subsequent parotidectomy showed a salivary duct cyst w metaplastic change (note the FNA tract here) Amylase crystalloids are assoc'd w benign non-neoplastic processes (such as oncocytic cysts) and benign neoplastic lesions (Warthin tumor) 1/3 #CytoPath #ENTpath #PathTwitter

However, if the oncocytic groups r rare, I would only call atypical w a note stating it could be an onocytic cyst or a benign neoplasm (ie WT)& correlate w radiology Overall, amylase crystalloids r a very reassuring finding! Check out our prior article👉 pubmed.ncbi.nlm.nih.gov/33296146/

Had my first counseling meeting (virtual) with the Transition Readiness Program today. Starting one year out. Went over the way ahead and set up my next step: 90 min class in person with TRS. Will walk-in for that class in next 2 weeks. Honestly a little anxious about separating.
