This may be long. Cancer is an interesting topic because sometimes there is an opportunity for public health intervention People can reach late-stage ( stage 4) cancer without “noticing” because not all cancers announce themselves early. It depends on the organ and the behavior/type of cancer. A couple of examples ——- Kidney Cancer The kidneys sit deep in the back of the abdomen. Small kidney cancers are usually not visible or palpable on physical exam. Many cause no symptoms early. There is also no recommended routine screening test for kidney cancer in average-risk people. So it may only be found when it grows, bleeds into urine, causes pain, or is seen accidentally on imaging done for another reason. —- Liver cancer The liver has a large functional reserve. That means a tumor can grow while the remaining liver still does enough work that the person feels “fine.” Early liver cancer may cause no symptoms, and when symptoms appear, they are often vague: weight loss, poor appetite, fatigue, abdominal discomfort, and nausea. Also, liver cancer often develops in people with chronic hepatitis B/C or cirrhosis. But many people do not realize they have these conditions until it is too late. That is why screening and vaccinations for hepatitis B are crucial. —— Now compare that with cervical cancer or colorectal cancer Cervical cancer can often be prevented or caught early through HPV (Human Papillomavirus testing and Pap smears. Colorectal cancer screening can find precancerous lesions before they become cancer. That is the difference. Some cancers have effective population screening pathways. Some cancers have early visible or testable warning signs. Others are deep, silent, nonspecific, biologically aggressive, or occur in people whose risk factors were never diagnosed. So, stage 4 cancer is not always personal negligence. ➤ Sometimes it is biology. ➤ Sometimes it is anatomy. ➤ Sometimes it is a lack of screening. ➤ Sometimes it is vague symptoms. ➤ Sometimes it is delayed access to diagnosis. The public health message is that: Know your risks. Use screening where screening exists. Do not ignore persistent unexplained symptoms. And build health systems that enable early diagnosis.
