Most people don’t know this but a patient can present directly to a Physiotherapist without a physician’s referral. This is known as direct access to physiotherapy, and it is a recognised practice in many countries, including Nigeria in appropriate healthcare settings. Physiotherapists are first-contact practitioners, meaning they are trained to independently assess, examine, diagnose within scope, and manage musculoskeletal and movement-related conditions. Where necessary, they can also refer patients to a physician or other healthcare professionals for further investigation or management if need be.

I was scrolling through TikTok when I came across this post and decided to give a bit of public education. The condition shown is CTEV – Congenital Talipes Equinovarus, commonly known as clubfoot. It is a congenital condition where a baby is born with one or both feet turned inward and downward. In clubfoot, the bones are normal. The deformity occurs because the muscles, tendons, and ligaments on one side of the foot are shortened and tight, leading to the equinovarus position. In infants, surgery is not always the first option. Their tissues are highly flexible, and with early referral and intervention, physiotherapy can gradually correct the deformity without cutting. Physiotherapy management focuses on gentle stretching and mobilization, casting, muscle reeducation, muscle strengthening, patient education on home positioning, gait training and amongst other modalities for pain management if involved etc. Consistency matters more than force. Correction is gradual in this case, it took about 3 years. It’s not magic.
