The Collaborative Care teams have been awarded the Airedale Partnership Award for working “creatively, enthusiastically & collaboratively to enhance our place-based Urgent Community Response model & offer to the population of Airedale, Wharfedale and Craven.” #PrideOfAiredale🌟

Day 6 of #AiredaleAdvent - our community advanced care practitioners team This is Fiona, Linda, Jim and Pauline. We love the festive glasses and antlers! This team provides advanced clinical assessments to patients in their own homes and also support Castleberg hospital.

This is why I absolutely love my job and am so proud of my Ward 6 team. This couple are both patients on our ward, she is very unwell and he wants to be by her side! So we moved them into sideroom so they could be together as they have been married 69 years!!💖 @AiredaleNHSFT
