Once upon a time a designer publicly swore at me (a customer in his shop at the time) on here because I highlighted the inconvenience of being asked to draw wads of cash to spend in his store because I couldn’t use my Amex card for payment..
One of reason he didn’t make provision to accept that payment option available?? >> “that type of card is easy to defraud/ use for fraud” etc..
What did Queen B say again? “Well… Weeeell.. weeeeeeelllllll…. What do we haaaave here….” Coz guess whose name I saw the SIU mention in the Lotto heist mess…??

@drkeithsiau This little shit (lol) also causes a lot of misery when local water supplies are low and we have a lot of sedimented water at the bottom of dams. Saw that a lot in the droughts in Cape Town, ZA, in 2017.

@sindivanzyl The mom was young and it was her first baby. She was giving baby a lot of gripe water BEFORE feeds. So baby was basically too full to feed.

@aribindi @dr_cellini In my country (South Africa), we do a lodox as they arrive in ER. That way, IF the bullet is still present, we have a quicker decision making process. Bearing in mind of course, our countries have different protocols. 👍🏼

If you have one hole in the belly, and the bullet is in the right leg, your algorithm for what to do just changed dramatically. Same if the hole is in the R chest and the bullet is in the L chest.

Keeps my (lack) or skills up for when I need that resus line too. I get what you're saying tho. Usually get that look from them!

Reckon a simple conversation with the surgeon can often result in an arm free. Does tucked = unsafe though? We do cardiac and robots with both arms tucked. Some circumstances probs safer?

@aribindi @dr_cellini Honestly, finding the bullet is the least of the patient’s concerns if they have a thoracoabdominal GSW.

@dr_cellini It's pretty useful in trauma- thoracoabdominal GSW, if there is no bullet in the belly, you know you better find out what cavity it is in-

@dr_cellini 30 years ago in an American ER (I was on my elective as a med student) I saw a hypovolaemic trauma patient w/ massive extremity trauma, in PEA, taken to CT 🤔 as the “bleeding had stopped” 😳 #AllBleedingStopsEventually

@DrJenGunter Currently 1USD = 19ZAR; so would be pretty affordable if not exceptionally marked up, which happens in many retail pharmacies. Available for free at government clinics, though.
