We often take breathing for granted, but what if one day, those 23,000 breaths dwindled down to a desperate struggle for just one?
Don’t wait. Get vaccinated.
Because every breath is worth fighting for.

@earlouismd @philchestorg @RespirologyAPSR @pccp_lunginfexn @gdyagra @Docrsp @CDelaCruzMed @CatiaCilloniz @ericmoral It’s appropriate to step down once the patient is clinically improving (afebrile, hemodynamically stable, less/resolved SOB, & oxygen needs are decreasing) and/or they’ve completed a short, guideline-based (usually 5-7 days) course of antibiotics #PneumoniaChat

@earlouismd what if it's not pneumonia in the first place? Pneumonia vs Congestion is a frequent battle. we should also advocate for Abx stewardship across diff subspecialties #PneumoniaChat

A3: Use antibiotics wisely by 1) starting broad *only* when bacterial pneumonia is likely, 2) narrowing once cultures, antigen tests, and/or clinical data clarify the cause, & 3) stopping when the patient stabilizes, fevers resolve, & a 5-7 day course finishes #PneumoniaChat

@earlouismd Clinical parameters such as resolution of fever, improvement of symptoms: less cough, less dyspnea; decreasing O2 requirement are signs that the patient is improving. Recent studies show no add'l benefit with longer Abx tx duration: 5days should be enough #PneumoniaChat

@earlouismd we need to stand firm and be courageous to not give in to the request of antibiotics prescription when not needed #PneumoniaChat

@Docrsp Elephant in the room would be Macrolide overuse. I think we still have a long way in terms of over accessibility. Physicians giving in to requests for azithromycin with no indication #pneumoniachat

Q3: Fighting Resistance
We have to stop using antibiotics incorrectly to save them for the future. When you start antibiotics for pneumonia, how do you make sure you are using them wisely, and when do you decide to stop or switch them? #PneumoniaChat #WiseAntibiotics

Q2: Oxygen Access
Oxygen saves lives, but it's hard to get in many places. What simple, practical ideas or equipment changes have helped you get and manage oxygen for your patients, especially in smaller clinics or rural areas? #PneumoniaChat #OxygenSupply

@earlouismd in pneumonia management, Non‐invasive ventilation can reduce the risk of death in the ICU, endotracheal intubation, shorten ICU stay and length of intubation. cochranelibrary.com/cdsr/doi/10.10… #PnuemoniaChat

Q1: Better Diagnosis
We need faster ways to diagnose pneumonia! Aside from the usual X-ray, what quick tests or tools are you finding useful right now to figure out if pneumonia is bacterial (needs antibiotics) or viral (doesn't need them)? #PneumoniaChat
