🎖️The scientific outcome of our epiphrenic diverticulum case — presented at #DDW 2025, San Diego and honored with the Best of the Best award — has now been published.
💥This work represents more than a technical description; it reflects an effort to rethink the pathophysiological foundations behind therapeutic decision-making.
In the light of the principles of Pascal, Laplace, Bernoulli, and Poiseuille, we explored how pressure balance, flow dynamics, and wall tension can guide endoscopic strategy.
☄️Different from our previous experiences, in this case we shared an approach performed without septotomy, aiming to restore intraluminal esophageal pressure equilibrium while enabling esophageal reconstruction.
🔭The physiological rationale, scientific framework, and clinical implications of this concept are discussed in detail in the discussion section of the manuscript.
💡In addition, we believe that integrating suturing strategies into advanced endoscopic resections may represent more than a technical step — potentially accelerating a paradigm shift in therapeutic strategy.

@hananmalghamdi@drkeithsiau Post-ESD histopathology confirmed a final diagnosis of a completely benign lesion with clear margins. ESD provided both diagnostic clarity and curative resection. With this organ-preserving approach, the patient has been followed for nearly four years without any recurrence.
