@RupertLowe10 We have a regulatory obligation to ensure that we properly understand our patient’s symptoms and that patients and carers understand our medical advice. Family members can easily misinterpret - when a professional won’t. Mistakes cost lives and much more money than the service.

Not a penny should be spent on translation services into foreign languages within the NHS. They even actively discourage family/friends to do the translation - they enforce a taxpayer funded translator. This is INSANE. You live in England, you speak English. End of story.

@D_Rodders I think this was a quirk due to what the different colleges felt was appropriate for remotely supervised practice (and their linked “exit exam” timetables), but @RodMack2 will know.

Anyone know why the requirements for PHEM training is ST4 for EM and ICM trainees, but competition of ST5 for anaesthetic trainees?

@alistairsteel @D_Rodders Hi, the earliest that PHEM training can commence is after successful completion of the fourth year of specialty training (ST4). Base specialties may have different entry points beyond this in order for PHEM subspecialty training to dovetail most effectively with base training.

@alistairsteel It was a regular feature when we used ‘cards’ to record ED notes - with a range of fields / boxes that ought never to be blank, rules about signatures, ink colour etc. It was,m more of a process focused audit tool. Concept remains with clinical focus (10 patient review).

EM people, I think there used to be an RCEM recommendation of a "10-card audit" for reflective practice - is that still a thing?

@alistairsteel @rachelgemma90 Other than completing open fractures, we’ve only had one formal AKA in EoE over many years (2013) and there was time to mobilise surgical support. @LeechCaroline did some thinking on this before!

@sillyoldjen I think the system is wrong - and support NHS litigation reform. I also think the conditions which lead to mistakes (crowding, under staffing etc) need to be prevented and managed. committees.parliament.uk/committee/81/h…

@RodMack2 Do you think that not-wealthy patients who've been very seriously injured by #NHS medics & who can't work any more should be allowed to take early retirement & be able to get their state Pension before official pension age instead of having to beg for DWP #disability benefits?
