@mshlcurrie @dp_oneill @dc_cavanaugh @alikhan28 @cmonsen @brandonksim When PCP (other physicians too) takes total cost of care risk, most health plans, including CMS, shares patient level claims history. Diagnosis, etc. are important for continuum of care. Besides, no other effective way to manage total cost of care.

There are many things that accountable/risk-bearing PCPs need to measure and manage that cannot be calculated purely from data contained in that PCP’s own EHR. That’s true even for measures of clinical quality, not just for cost.
