Exposed mains live conductors on a compressor; recipe for electrocution or electric shock incident. This was taken out of use an during inspection of tyre fitters.

Found during a recent H&S inspection. Make sure your inspection pits are suitably covered or have barriers around so as to prevent people falling into it.
