6.4 Next, plans for the Neurotrauma #CarePathways Phase 2 (2022-23). #NCP

6.3 The Data Prioritization project identified indicators to evaluate & improve #neurotrauma care. 22 core indicators to address insufficiencies in the community sector and patient characterization, and value health equity and patient/caregiver education. #NCP

6.2 The Ideal Pathways Project developed evidence- and experience-based care pathways for mild-TBI/#concussion, Mod-Sev #TBI, and traumatic #SCI. The Pathways are comprehensive of neurotrauma care necessities, need for re-access, caregiver burden, and individualized care. #NCP

6.1 In Phase 1 of the Neurotrauma #CarePathways Project, we evaluated the current state of neurotrauma care, updated guidelines, and developed Ideal Pathways and data indicators to address the inadequate #neurotrauma care system in #ON. #NCP 🧵

5.4 The aim of the project was for #ON to: #NCP 1) Collect appropriate descriptive data about individuals with #TBI and #SCI 2) Improve data integration between publicly funded and 3rd party care providers 3) Improve quality of data collection throughout the care continuum

5.3 67 data indicators were identified. 42 were designated as ‘must-have’ indicators. 22 ‘must-haves’ were selected as a core set (1 Pre-Acute, 7 Acute, 4 Rehab, 7 Community, and 3 Critical Considerations). #NCP

5.2 The Data Prioritization project aims to improve #neurotrauma data collection and integration in #ON. A diverse group of stakeholders identified and developed quality of care #indicators, addressed gaps in data collection, and mapped indicators onto the Ideal Pathways. #NCP

5.1 The Ideal #CarePathways will only be successful with appropriate evaluation strategies; this is where the Data Prioritization project comes into play. #NCP 🧵

4.10 The Ideal Pathways will also contribute to improved #healthcare provider confidence in the system, reduced inappropriate use of services, and mitigation of comorbidities and preventable complications, thus promoting holistic care and efficient use of resources. #NCP

4.9 The Ideal Pathways will align with the #QuadrupleAim for healthcare where #patient & #caregiver experiences are taken into consideration during care planning, navigation, and care provision, resulting in #equitable health outcomes. #NCP

4.8 Critical Considerations include geographic location, marginalized populations (e.g. #Black, #Indigenous, #LGBTQ2S+), income/housing/food security status, #mentalhealth issues, and availability of caregiver support. #NCP

4.7 The Critical Considerations Working Group targeted care needs for vulnerable individuals & developed elements that address gaps in current #CPGs and public access to care. #NCP
