Z-POEM in an woman 84-year-old with a 2 cm Zenker diverticulum and thick septum. Very symptomatic (DRC score 4 points). The intervention is performed with Hybrid-Knife T and VIO-3 (endocut I 1, 2-2-2; Precise sect 4.5). The mucosectomy was closed with 5 lockado 11 mm clips.

Elderly Patient with ESLD, with fevers, chills, white count, and ruq abdominal pain. US showed signs of cholecystitis as did HIDA. IR was concerned about ascites and surgery declined due to being a poor surgical candidate. We did EUS guided GB drainage.

@ENDOS96 As far as I can understand, the working angle is between a standard EUS scope and a standard duodenoscope. That may be a little problematic as the bleeeding here is caused by the too deep cut because the sphincterotome could not be bended.
