I always wanted to do this. With enthusiastic consent from a patient, here's an unedited cataract case with annotations and narration with the "hand view". I'm proud to share what I've learned from my awesome mentors so far. youtu.be/osVvjEt9AlM #ophthalmology #ophthotwitter

@EyeFlyMD Shows strength of LASIK even today as a one-stop solution, basis refractive error, when one has to pick a single procedure 🤔 Can you share the book buying web address as well?

@DrBradBarnett I know a few RLE but agree trifocal is rare. IMO my favorite application of RLE is the 30 year old +7.00 that would not do well with LASIK.

@EyeFlyMD @EyeFlyMD food for thought. Do you know of an ophthalmologist that has undergone a clear lens exchange? What percentage of ophthalmologists get trifocal lenses themselves? With exchangeable optics on the horizon do clear lens exchanges in anyone under 45 make sense ?

“Which IOL is the best?” I’m lucky to have gotten pretty extensive experience with essentially all of the modern premium IOLs in 2024 and plan on doing a deep dive video early next year. Very impressed with Odyssey and Envy so far. #ophthalmology #optometry #eyes #cataract

SMILE is an awesome procedure but enhancing it can be tricky. Here is our “cap-to-flap” method to convert a SMILE cap into a LASIK flap. #ophthalmology

@eyemd76 For the basic cases I always make a C shape like this. It both radializes it and flips it over in one motion.

I changed how I start my capsulorhexis this year. It's now one continuous motion with the cystitome rather than multiple attempts to flip the leaflet over. More predictable and efficient. Full video: youtu.be/E_9RT9dwNsE

@EyeFlyMD @JotaCastro96 I would argue that inferior like this is THE BEST place for iridotomy. Less pain and less dysphotopsia than nasal or superior

While uncommon, we do everything at PNV including the occasional LPI. With state of the art imaging I’m able to record super HD videos like this. Here’s an LPI for narrow angles in the IT quadrant. #ophthalmology

Today marked 1,000 career cataracts, start-to-finish, all by myself, excluding RLEs. To hit this milestone 4 months out of residency is incredibly special. Thanks to Al my mentors and Dr. @gregparkhurstmd for making this possible. #ophthalmology

@JotaCastro96 I start at 4. I like IT because it’s so easy to see and the literature shows there’s no association between PI location and dysphotopsuas.

@EyeFlyMD With what power do you usually star? Why the preference for IT quadrant?

@EyeFlyMD What’s the imaging system/hardware used here? It’d be super useful to take such crystal clear videos

Great example of nuance. This “high” vault is perfect for this eye. Learning the nuance of sizing prediction is one of my favorite things to learn from Dr. @gregparkhurstmd. ICLs are my favorite procedure by far. #ophthalmology #ophthox

Incredibly special moment today as I got to be my wife’s #EVOICL surgeon! 20/15 an hour later with great IOP. Thanks to Dr. Parkhurst and the entire PNV team for making this such a memorable experience. #ophthalmology

I don’t think any other part of cataract surgery is more important in making the rest of the case go smooth than hydrodissection. #Ophth almology
