@Melissa_S_Ryan @medicalmodelbri I don’t disagree with you. I’m very much in favour for fighting for more doctors. I didn’t push for the development of the ACP role and I don’t think it should continue to be expanded. But I don’t think existing ACPs should be sacked. That’s my stance in one tweet.

Ok, the department is short of an EM consultant . If it’s a choice between an ACP and a gap , would you rather have an ACP? Let’s talk through this logically . Cant be because at that level you need someone with MBBS because any experienced ACP can work as a Dr without MBBS

@jim_crawfurd @medicalmodelbri To exception report it as ‘lack of clinical support’. And if there are multiple gaps or it’s happened on multiple days to report patient safety concerns. Under the new ER system, all rota gaps of all shift types must be reported by the guardian of safe working (3/n)

The shift is either not filled or filled by non-doctor staff (possibly at higher cost). Next step is to have every resident doctor on the floor covering the staffing gap (2/n)

@rut85826 @DocEmUK @ExMomentum_NHS This would probably be a category 2 ambulance response - average response time in Eng was 29m in Feb 2026. But they then need to convey to hospital as well and get triaged/seen in ED. Definitely a slipping standard of care.

@DocEmUK @ExMomentum_NHS It’s a terrible thing if they don’t come out immediately for an unwell baby. I found the Scottish ambulance service generally very good especially with children. Things just seem to be going downhill.

The systematic undermining of general practice has consequences that you can't even imagine. When #1 was 2 weeks old, he developed a fever, was grey and floppy with a bulging fontanelle. I called the GP OOH and he was seen within half an hour of my call. IM benzylpenicillin and a week in hospital. That OOH GP saved his life, no question. He has just been offered a place to do an MSc in mathematical and theoretical physics at Oxford - unbelievably competitive. Do I think today's 111 would have achieved the same? Not a chance.

Your solution: Force GPs to deal with unlimited numbers of patients every day. Reduce waiting lists by allowing hospitals to reject GP referrals. Share confidential GP records with private companies. I’ve never known a health secretary show such hostility to General Practice.

Honestly so sick of this crap The GMC needs abolished ASAP A PA is investigating doctors at the GMC. They no longer are on X and have successfully avoided criticism for months. We continue to pay extortionate fees to them like good little rats. This has to change.

A PA graduate who describes their qualification as “Medicine” is now employed by the GMC to investigate doctors’ fitness to practise. They are not themselves registered with the regulator they serve. You genuinely could not make this up.
