🚨 𝗕𝗥𝗘𝗔𝗞𝗜𝗡𝗚 from #ACC2026: Two major RCTs just challenged everything we thought about Impella pumps in cardiac care. The results? Surprising. The implications? Massive. A thread on what every cardiologist needs to know 🧵👇

A low-cost and expert-driven medical technology is being used in rural areas in Australia! The machine is operated by a sonographer remotely (using a gaming controller) and helps perform an ultrasound examination. Doctor shortages shouldn't mean that patients have to travel more (sometimes for no medical reason), but to use technologies that can extend the reach of medical care. This is a perfect example of that!

You know what’s financially profitable? Treating someone for liver disease throughout their life (Hepatitis B is incurable). Know what’s not? Giving them a cheap vaccine at birth that prevents it all.

Contrary to much of the nonsense you may read on here, mRNA vaccines don't cause deaths - they prevent them This massive study from France showed that not only did mRNA vaccines not increase mortality but they reduced deaths and saved lives

🚨 NEW: One of the largest COVID-19 vaccine studies ever (28 MILLION people in France) just dropped. Results? • 74% lower risk of death from severe COVID-19 • 25% lower risk of all-cause mortality • No increase in 4-year mortality • Vaccinated people had lower risk of death from any cause. In short: mRNA vaccines weren’t just safe — they were protective across the board. 🔥📉

HK does have a housing issue but the place that is set on fire isn’t “public housing” in the way it’s understood in the West (or even in Hong Kong). I don’t blame the Ms Ren for the error - it’s yet another eg of Western media not appreciating the nuances behind housing in Hong Kong when reporting on the fire. The place where the fire started is called Wang Fook COURT, which in Hong Kong parlance denotes residential housing construction of which is arranged by the government to be sold below market price to lower middle income earners, but primary buyers who decide to sell at a later date must first pay the difference between purchase price and the place’s market price at time of purchase. In other words, the Government effectively gives interest-free loans to the first hand buyers of such apartments with terms that end when the first hand buyer sells. By contrast, public housing as the term is typically understood (ie government builds and people rent at lower than market rates) are another category of housing altogether, and in Hong Kong they are typically called “[Name] HOUSE”. While the HK Government has tried emulating Singapore and letting long term tenants buy their own public housing units at below market rates, the take-up hasn’t been great because these apartments are subject to a whole lot more restrictive covenants as to the “enjoyment” (in the property law sense of the term) of them, compared with the COURT apartments that get sold.
Liqian Ren@liqian_ren

So sad. Most headlines also don’t mention these are HK public housing. HK housing price is so high yet housing price is mostly supply constraint by the developers. HK doesn’t lack land to build and lower housing prices, just lack political will.

@venkmurthy I did MDcalc's evidence appraisal for it and essentially found that whilst calibration is poor, NPV is good. Controversial perhaps, but it shd be a tool to give reassurance for not giving anticoag in cases with low scores, but not as the only criterion for initiating them.

CHADS-Vasc is among the most used risk scores in cardiology. This not because it is most accurate or best calibrated but because it allows docs to feel less queasy while prescribing anticoagulants, one of the highest risk categories of medicines Offloading the cognitive load!
John Mandrola, MD@drjohnm

Please look at Gene Quinn et al paper. The problem and this is warning. You can’t unsee the massive variance in stroke risk at a set CHADSVASC score. ahajournals.org/doi/10.1161/CI… Read paper. Save it. CHADSVASC calculator = folly
