A 39-year-old patient was referred for an asymptomatic retinal lesion in the left eye. Fundus examination (A) revealed a well-circumscribed, whitish intraretinal mass located along the inferior temporal arcade (white arrow), associated with adjacent preretinal neovascularization (green arrow). Autofluorescence imaging (B) showed a homogeneous, round, hyperautofluorescent lesion, with a raspberry-like heterogeneous intraretinal mass on OCT B-scan (D). Fluorescein angiography (C) revealed homogeneous staining of the lesion, inferior temporal retinal ischemia, and preretinal neovascularization, suggesting a branch retinal vein occlusion secondary to a mechanical mass effect. ow.ly/UvJP50Yz9Hp #ophthalmology #retina

A 55-year-old healthy man noted 3 days of blurred vision left eye. He denied neck trauma or any type of neck procedure. Visual acuity was 20/25 right eye and 20/40 left eye. Bilateral optic disc edema was noted on examination (A). Magnetic resonance imaging and cerebral angiography confirmed a left external carotid artery-jugular bulb arteriovenous fistula (B, arrows). After coil embolization (C, arrows), the papilledema resolved and was replaced by pallor (D). Spontaneous jugular bulb arteriovenous fistula is a rare occurrence; however, when it occurs can result in elevated intracranial pressure due to cerebral venous hypertension manifesting as papilledema. ow.ly/BMZZ50Yz9AV #ophthalmology

A 36-year-old man was referred for management of a painless mass of the ciliary body. On examination, a nonpigmented conjunctival nodule (A) was present external to a lesion arising in the ciliary body that extended to contiguous choroid (B, ultrasound). Biopsy of the conjunctival tumor revealed histiocytes with phagocytosed lymphocytes (emperipolesis) (C, yellow arrow, ×200). Histiocytes expressed CD68 and S-100 (D, red arrows, ×200) and did not stain for CD1a. BRAF V600E mutation was not present. The diagnosis was extranodal Rosai-Dorfman disease, a rare non-Langerhans cell histiocytosis. ow.ly/mWWi50Yz9uX #ophthalmology

A 61-year-old man presented with obscuration of vision in the right eye due to a slowly progressive, painless lower eyelid swelling of 7 years duration. (A) Oblique clinical photograph showing an 8 × 4 × 3 cm firm-to-hard pedunculated lesion arising from the right lower eyelid with overlying dilated vessels and pinchable skin, causing visual axis obstruction. Best-corrected visual acuity was 20/20 in both eyes. (B) Computed tomography demonstrating a polypoidal soft-tissue lesion with calcifications in the inferior right periocular region. A 55-year-old man presented with progressive vision loss in the right eye (OD). Best-corrected visual acuity was 20/400 OD and 20/20 in the left eye. Anterior-segment examination revealed numerous superficial, centrally located brown deposits OD (A), raising concern for primary corneal melanoma. After fluorescein staining, a retained contact lens with pigmented deposits was identified and removed (B). Microscopic evaluation revealed lightly pigmented reflective fungi on the lens surface (arrows) and penetrating the lens (arrowheads), associated with pigmentation of the lens (asterisk) (C; unstained preparation, ×400).
