A 57-year-old woman had cataract surgery in her left eye. On postoperative day 1, a live worm was observed in the anterior chamber (A, red arrow) that quickly migrated into the vitreous cavity. Another worm found in the conjunctival sac (A, yellow arrow) was removed and identified as Thelazia callipaeda. At follow up, the worm in the vitreous cavity was observed (B), removed via vitrectomy, and confirmed as T. callipaeda (C and D). The patient admitted intimate contact with dogs and cats. Thelazia are normally found as multiple worms in the conjunctival sac. We hypothesized the worm entered the eye through the unhealed corneal incision.
