A neonate born at 34 weeks of gestation weighing 1600 g, who underwent initial resuscitation with 100% oxygen and total oxygen administration for 17 days, was screened for retinopathy of prematurity (ROP) at 37 weeks of postmenstrual age. Examination revealed severe plus disease, arteriovenous shunts (white asterisk), flat neovascularization in zone 1 (yellow asterisk), and obliteration of major vessels (black arrow) in the periphery. A, B, Administration of unblended oxygen injures immature capillary endothelial cells and results in capillary obliteration, followed by relative hypoxia upon shifting back to room air. This can explain severe ROP in large preterm infants from developing countries, simulating oxygen-induced retinopathy. A 38-year-old woman presented at 28-weeks’ gestation with 1 month of blurred vision (20/25) in her right eye. Examination revealed a circumscribed choroidal hemangioma (A) of 3.8-mm thickness with subfoveal fluid on OCT (B, C). Treatment was deferred until after delivery. Two weeks postpartum, the hemangioma (D) was thinner at 3.5 mm, the subretinal fluid resolved (E, F), and vision returned to 20/20. During pregnancy, choroidal hemangioma can leak subretinal fluid and become symptomatic, secondary to hemodynamic changes and plasma volume expansion. The subretinal fluid can resolve spontaneously after delivery. Hence, conservative observation is recommended. A 14-year-old girl presented with bilateral vision loss for 3 months. Ultra-widefield swept-source OCT angiography (BM-400K BMizar, TowardPi Medical Technology; 160°) revealed extensive areas of nonperfusion, particularly involving the macular area (A, D). Widefield B-scans (24 mm) of both eyes demonstrated no significant macular structural abnormalities (B, E). However, flow within the retinal vessels of the central inner retina was reduced, corresponding to nonperfusion areas in the central macula (C, F). Beta-thalassemia was confirmed by genetic testing, clinical features, and family history. The patient underwent hematopoietic stem cell transplantation 3 months earlier. Retinal perfusion should be monitored even after transplantation to prevent irreversible vision loss.
