PVC localization isn’t guesswork, it’s anatomy translated into ECG patterns.
This image maps premature ventricular complexes (PVCs) arising from the outflow tracts (RVOT/LVOT) and tricuspid annulus, linking exact anatomical sites with their ECG signatures.
🟢 Core concept
PVCs from RVOT and LVOT generate inferiorly directed activation → dominant R waves in II, III, aVF
🟣 RVOT origin (anterior structure)
Depolarization moves away from anterior chest wall
Produces qS or minimal r in V1–V2
Typically shows late precordial transition (>V3)
Can have right inferior axis (especially leftward RVOT)
⚪ LVOT / Aortic cusps (posterior relative to RVOT)
Depolarization moves toward V1–V2
Leads to higher r/S ratio in V1–V2
More posterior sites (LSOV, GCV) show larger initial R
Usually left inferior axis
🔴 High-yield distinguishing clues
RSOV–LSOV commissure: notched downstroke in V1
Great cardiac vein (epicardial): Q in aVL/aVR >1.7
RVOT free wall: notching in II, III
🟡 Special regions
Para-Hisian: I, aVL show R; aVR shows qS with QRS <140 ms
Tricuspid annulus / His region: more inferior → horizontal axis shift
Big picture:
Anterior vs posterior origin shapes V1 morphology
Right vs left origin determines axis
Proximity to conduction system influences QRS width
Master this, and you can localize PVC origin before entering the cath lab.
