This is the cognitive dissonance that underlies the conflict. On the one hand you can top up and do ED. On the other doctors can't top up and do paramedicine. It is a perverse situation where you can only see skill mixing as appropriate in one direction. And this likely speaks to the fact that you may well be a very good paramedic where you can appreciate the skills and strengths required under the pressure, stress, and dynamism of the specific breadth of clinical decision making you may need to make in the clinical settings paramedics find themselves in. But does this expose a lack of understanding or appreciation for the need of broad based medicine in a hospital setting? The access to wider investigations (and clinical reasoning for using them), the interpretation of data in clinical context, the ability to coherently consult with specialists, and/or the ability to communicate with GPs.

Performing an endoscopy without supervision does require consultant/SpR level knowledge & skills with extensive understanding of anatomy, physiology, pathology, management of pre, intra and post op complications. It is not just a simple task..

It certainly does not require consultant levels of pay. Lets follow this through to its ridiculous conclusion only consultants can take height, weight and blood pressure checks

Other than when anaesthetists split away from surgeons as a specialty and when the intensivists decided to split away from us, I don't think there has been an issue that anaesthetists have felt so strongly about with reference to their professional identities as the issue of AAs.

The PA who totally misdiagnosed Emily Chesterton which caused her death IS still working at a different GP Surgery to do locum PA work because she refused to have tighter supervision!

Judge has recognised our case gets "right to the heart of the matter" and it may need to be extended for an extra day. Key point our team made was that the decision by the GMC to not set scope was reached with no identifiable decision-making process. And this despite the GMCs own survey, echoed by many others expressing concerns over scope, consent and supervision. #AUvsGMC

Often I don't comment on twitter, BUT the UK high court ruling today saying there is no confusion between doctors/physicians and PAs (Physician Associates) by misspelling on the first page @RCPhysicians calling them the Royal college of PAs is embarrassing beyond belief!
