Can modern external ring fixation improve recovery outcomes in severe open tibial fractures? In this secondary analysis of the prospective randomized FIXIT trial, Shu et al. found that patients treated with modern external ring fixation experienced significantly greater pain interference at 6 months compared with internal fixation, although differences resolved by 12 months. A key secondary finding showed that patients with retained external fixation frames at 12 months reported substantially greater pain intensity and pain interference. These findings highlight the importance of postoperative pain management and frame-related care when utilizing external ring fixation in severe tibial trauma.
