@abalajontoto @SocietyGURS @ISORU1 Because he has had APR, this is a fistula to a cavity where his rectum used to be, right? Can do buccal+gracilis. Cystectomy as a salvage operation if that doesn’t work.

Deep rectourethral fistula, just proximal to the veru. The rest of the urethra doesn’t look good either. Patient previously underwent abdominoperineal resection for rectal Ca, thus he no longer has an anorectal opening. Any treatment suggestions? @SocietyGURS @ISORU1

Just finished fixing this fistula from Indiana Pouch to urethra in man with h/o XRT. Small pubectomy, difficult dissection off rectum, primary closure of pouch hole; good omental flap. No reason to throw out a good pouch and convert to a conduit.
