Great list. Point 6 (Missing human escalation path) and Point 8 (Irreversible action) are exactly why we built ActionLayer. When agents execute critical web tasks or make purchases, you need an infrastructure that pauses the action and asks for human approval on a phone before spending money.

Every way I've seen AI agents fail in production. Bookmark this before you ship 1. Silent context truncation mid-task 2. Tool call with no timeout 3. Hallucinated tool output treated as ground truth 4. No idempotency on retried actions 5. Cascading replanning loop 6. Missing human escalation path 7. Stale session state resumed as fresh 8. Irreversible action with no rollback 9. Blast radius from over-permissioned tools 10. Confidence collapse on ambiguous input 11. Prompt injection via tool response 12. Cost spiral from unbounded retries 13. Cross-agent state desync 14. Observability gap at LLM hop 15. Kill switch that requires redeployment All of these are architecture failures

@ai_for_success the asynchronous worker flow is the future. let the user keep talking while the agent executes in the background. wiring ActionLayer into a setup like this would let the agent actually buy things and navigate dynamic sites while you just chat. great build.

I love Hermes Agent and Gemini Live is also one of my for voice conversation, so over the weekend I built a small vibe-coding project that brings the two together called IRIS. The idea is simple: Gemini Live becomes the conversational interface, while Hermes Agent handles execution, automation, and long-running tasks behind the scenes. I can talk to Gemini naturally, and whenever something needs real work, Gemini formulates the request and hands it off to Hermes. The best part is that the conversation never stops. Gemini keeps talking with me naturally while Hermes works in the background. When Hermes finishes, Gemini proactively tells me the results are ready, switches context to the Hermes output, helps me review it, and then we return to our normal conversation. That manager/worker flow feels really good. Because I wanted the app to be as hands-free as possible, I also added a camera-based gesture layer using MediaPipe Gesture Recognizer. I can point at Hermes task cards, open details, scroll through results with my hand, and close them using gestures. Small weekend project, but I had a lot of fun building it. Also, the anti-flicker macOS app I vibe-coded yesterday ended up being incredibly useful for this project. It fixed my webcam flickering issue, so I can finally record videos properly. 🙂

@petergyang the paranoia is justified. agents usually create slop when they get stuck trying to navigate dynamic UIs they don't actually understand. if you give them a native execution infrastructure to handle the web friction, the loops actually become reliable.

I am pretty skeptical of building loops for everything because I'm paranoid about the agents creating slop if I'm not there to monitor what they're doing. What are some good loops to try for coding or regular knowledge work?
