This report explores how a #PrimaryCare team piloted an AI tool to draft responses to patient portal messages, helping reduce inbox burden. Read full report here: annfammed.org/content/24/1/78 After early challenges, the team improved responses by refining prompts to better guide tone, safety, and content through message categorization and symptom-based tailoring. The findings show AI can support patient messaging but only with adaptable prompt design, up-to-date resources, and ongoing clinician input.

Continuity with a General Practitioner and Nurse Linked to Fewer Hospital Visits in Home Care Patients Read the full study: annfammed.org/content/24/1/17 A study of 1,207 older adults receiving home-based primary care found that patients who saw the same general practitioner for at least 75% of their visits experienced significantly lower use of acute care services. Higher continuity with a GP was associated with a 31% lower hazard of home ambulance use, a 39% lower hazard of emergency department visits, and a 35% lower hazard of hospital admissions.

𝘌𝘢𝘳𝘭𝘺 𝘈𝘤𝘤𝘦𝘴𝘴 𝘈𝘳𝘵𝘪𝘤𝘭𝘦: This study examined whether integrating social risk decision support tools into electronic health records could improve chronic disease management and increase social risk–informed care in community clinics. Full article here: doi.org/10.1370/afm.25… In a cluster randomized trial across a large primary care network, clinics using EHR-integrated social clinical decision support tools showed greater improvements in blood pressure control and increased social risk screening and documentation compared with control clinics. These findings support embedding social risk data into clinical workflows to enhance chronic disease management in primary care.

In this study, researchers from the @umfamilymed examined how patients value trade-offs between quick portal messaging, getting a visit sooner with any available physician, or waiting longer to see their own primary care physician Read full original research article here: annfammed.org/content/24/1/25 In a survey of 2,268 adults, most patients preferred a 3-day portal message from their own primary care physician over waiting longer for video or in-person visits. If messaging wasn’t selected, they chose a faster video visit with another physician.
