There is NO evidence that a keto diet increases the risk of heart disease. What there IS emerging evidence for is the potential metabolic & mental health benefits of #TherapeuticNutritionalKetosis. These alone merit future studies on its use as a medical treatment.

Do the authors already have their minds made up that high LDL is high risk, regardless of context? I hoped they would encourage more research into the "true" risk for these specific patients, especially those who don’t have existing plaque.

A new JAHA paper on high LDL in lean keto patients (LMHR phenotype) makes a curious omission: it never calls for studying hard outcomes like plaque progression. As a cardiologist, I have to ask, have they already made up their minds? 🧵

The goal shouldn't be finding "one best diet" for everyone. It should be helping each individual find the pattern that works for their specific metabolism, culture, and goals. Sustainability is the combination of clinical health improvements and personal preference.

@bschermd You ran a study on exactly this population. The lipid numbers produced median plaque progression at one year. The documentary does not mention that part.
