Dear #Neotwitter, ⁦@CardioNeo⁩ - are you a #NICU that practices LISA or INSURE in extremely preterm infants? Have you ever seen #pulmonary hemorrhage soon after #surfactant ? Does having a higher mean airway pressure after surfactant reduce risk of pulmonary hemorrhage?

Interesting moment in the NICU today… During a team conversation, someone brought up a point I had never considered before: For babies who develop a spontaneous pneumothorax shortly after birth, it turns out there are recommendations out there that these individuals should never scuba dive—for the rest of their lives. That really caught me off guard. While I understand that the risk of recurrence may be elevated in the first few years, why would this recommendation still hold 10, 15, or 20 years later? I have not reviewed evidence recently on this topic and wondered if I was alone. So I’m genuinely curious: 🔹 Have you seen these recommendations in practice? 🔹 Do you counsel families about this long-term risk? 🔹 Is there solid evidence behind the lifelong restriction? This feels like one of those niche but meaningful areas of anticipatory guidance that could have a real impact. I’d love to hear what others think or if anyone has literature to share. Thanks!
