Great job @fouadsjaber; idea came to me after I was referred a patient for G-POEM due to refractory gastroparesis after Nissen fundoplication. I had not observed similar outcomes in my cTIF patients. Data shows incidence of gastroparesis is similar, lower prokinetic use with TIF.

Today was the inaugural day of @bcm_gihep Advanced Endoscopy service in the St. Luke’s health- the Woodland hospital. I am grateful to Kalpesh Patel and @WasAbiDoc for their leadership on planning and executing the service expansion and my appreciation to the rest of Advanced Endoscopy team @FaresAyoubMD @KeihanianTara @GiJawaid for their enthusiasm to expand our sophisticated services beyond Texas medical center. Our group will provide inpatient and outpatients pancreaticobiliary coverage 365 days a year for our Woodland and North Houston community !

I’ve found using saline immersion to be helpful in differentiating the layers for careful dissection. Always wondered if patients with this dense fibrosis have different outcomes. Maybe we should look at this prospectively!

Can we safely offer colorectal ESD to elderly patients in Western settings? In our study of 710 patients undergoing colorectal ESD @StLukesHealthTX and @bcm_gihep , we found: 🔹 Comparable en-bloc resection rates across age groups: <65 yrs: 87.4% | 65–75 yrs: 80.9% | ≥75 yrs: 80.2% 🔹 Low and similar post-procedure adverse event rates: <65 yrs: 6.7% | 65–75 yrs: 8.5% | ≥75 yrs: 6.3% 🔹 Slightly longer hospital stay in ≥75 yrs (1.1 days), but still shorter than Eastern cohorts. Tell us more about your technique, did you use traction? An overtube for stability? Do your pathologists comment on deep and lateral margins - it took some communication and effort to have our pathology team here treat these like true oncologic specimens.

Building a program isn't just about being able to do a procedure. I was messaged asking about resection after this lesion was found on FIT+ colon. Years of communication, education, & discussion about what our #ThirdSpace program has to offer led to this patient having a minimally invasive removal & helped avoid R hemicolectomy. #OrganPreservingEndoscopy
