Pitch counts alone are failing to stop the UCL injury epidemic. UCL injuries keep climbing because we’re managing the wrong variable. Pitch counts only track in-game volume which accounts for 57.6% of total throwing workload volume. They ignore the 42% blind spot of total daily throwing workload—bullpens, warm-ups, long toss, year-round play. That’s where the hidden stress accumulates. Overuse isn’t just “too many pitches in a game.” It comes from a poor on ramp, poor recovery cycles or sudden spikes in intensity. Adolescent arms don’t fail from one outing—they generally break down from chronic load mismanagement. If you’re only counting pitches, you’re missing: • Throw intensity • Total throws per day/week - including pre-game and weekly throwing programs. • Recovery windows • Year-round workload trends That’s the disconnect.
