@stephanamayer I enjoyed this nimodipine review. Would love to see a nimodipine v placebo modern and large RCT. aSAH is so ripe for a large platform trial to test so many aspects of care. Eg. Milrinone, BP targets, IA therapy, lumbar drains, CTP guided intervention, EVD duration, Na+ mgmt etc!

@OGdukeneurosurg Realise there’s a move to unify it all as aphasia, but this loses nuance. And we don’t say aphagia. Or alexic etc

@OGdukeneurosurg I still prefer the term dysphasia for most of these. "dys” is a Greek prefix which means difficult, painful, bad, disordered, abnormal. “A-“ means total absence. So dysphasia is impaired speech whereas aphasia is total absence of speech reception or expression.

@DrAkhilX We use fludrocort and midodrine in ICU as oral alternatives to IV vasopressors in select patients with post operative vasoplegia. Never tried droxidopa for this though. Does anyone use midodrine and droxidopa combined?

Wow. In this paper in @StrokeAHA_ASA a panel of stroke neurologists was challenged with differentiating between human and AI authored review articles. Author type was accurately identified only 50% of the time. @Menon_Cambridge Will be interesting to see a phase 3 on this. To date we haven’t had much luck translating interventions that reduce radiological vasospasm into improved outcomes eg clasosentan. Maybe this will be different?

Don't worry if you can't find a cerebral aneurysm on angiography in perimesencephalic SAHs. No vascular cause is identified in up to 95% of cases. They are (presumable) venous hemorrhages. This also means 5% will have an aneurysm, AVM, fistula... #MedEd #RadEd #FOAMed #FOAMrad
