Leriche syndrome (aortoiliac occlusive disease) causes impotence by occluding the distal aorta and common iliac arteries. This blocks blood flow to the internal iliac arteries, which supply the penis via the internal pudendal arteries. Resulting pelvic ischemia prevents sufficient arterial inflow to the corpora cavernosa for erection.

✅Dx- Erasmus syndrome A stone/ceramic worker with dyspnea, pulmonary hypertension, and oddly clean fingertips. That subtle hand finding can point toward Erasmus syndrome silica-associated systemic sclerosis. Silica exposure → autoimmune activation → sclerodactyly + ILD + severe PAH 🔻The diagnosis is Erasmus syndrome systemic sclerosis (scleroderma) associated with occupational crystalline silica exposure, with or without radiographic silicosis. 🔻Management :- Remove from further silica exposure 🔻Immunosuppression for systemic sclerosis/ILD :- MMF, cyclophosphamide 🔻Treat pulmonary hypertension sildenafil/tadalafil, bosentan 🔻Supportive care:- oxygen, diuretics, pulmonary rehab 🔻Major prognostic factor: severe pulmonary arterial hypertension #MedX
