Retatrutide: 30.3% weight loss at 2 years. Triple agonist. Phase 3. We're entering an era where the molecule isn't the question... the right protocol, the right person, the right biomarkers to track is. And everyone in health, outside of the AI-sphere in going crazy. The mechanism story is bigger, and almost nobody will explain it today. TRIUMPH-1 headlines: → 70 lbs (31.7kg) average weight loss on 12mg at 80 weeks (1.5 years) → 45% of high-dose patients hit ≥30% body weight reduction (this is bariatric surgery territory) → 65% dropped below the obesity BMI threshold entirely → In the BMI ≥35 extension cohort, weight loss was still trending down at 104 weeks → even 4mg delivered ~19% loss with discontinuation rates below placebo → significant drops in blood pressure, triglycerides, non-HDL cholesterol, waist circumference, and hsCRP → no cardiac or liver signals Everyone will post the numbers today. Here's what most of them won't explain. The reason retatrutide keeps working when other GLP-1s slow down is the glucagon receptor. 1. Semaglutide is GLP-1 alone. 2. Tirzepatide adds GIP. Neither touches glucagon (a hormone produced by the pancreas that raises blood sugar levels by prompting the liver to release stored glucose). Both suppress appetite and improve insulin sensitivity: but neither directly drives energy expenditure the way glucagon does. Glucagon does three things that appetite suppression alone can't: 1. Tells the liver to burn fat directly 2. Raises resting metabolic rate 3. Suppresses new fat creation from carbohydrates That's why semaglutide and tirzepatide trials tend to plateau around 60–72 weeks. The body adapts. Metabolism slows. The drug becomes a brake on intake, not an accelerator on output. Retatrutide (a triple-agonist = a type of medication that simultaneously binds to and activates three different hormone receptors to regulate appetite, blood sugar, and metabolism) It keeps the furnace running on the output side while suppressing the input side. That's the mechanistic reason the extension data was still trending down at 104 weeks while everyone else's curves flatten. The likely downstream consequence — supported but not yet head-to-head proven: more visceral fat loss, better lipid improvements, more durable metabolic remission. The Phase 2 visceral fat data already pointed this direction. The bigger implication for our industry: "GLP-1" was never really a category. It was a placeholder until we had multi-receptor molecules that target the metabolic system as a system. Today's results make that real. Full press release in comments.

Now that we have phase-3 results for retatrutide, it's time to revisit the question: What would happen to obesity if we gave every obese (BMI 30+) person the drug for just one year? Well, the obesity rate would fall by more than 80%! We'd be skinnier than the U.S. circa 1980!
