Er fragt sich: Wenn sie alleine entscheiden darf, ob das Kind kommt, warum darf er nicht alleine entscheiden, ob er Vater wird? Ich bin ihm meine Antwort schuldig geblieben.

You're right, and I'll take that. Heart meds, thyroid, BP. If the mechanism is broken, you replace it. Indefinitely. That's just medicine doing its job. Where I'm landing is that obesity isn't one thing. For some people it absolutely belongs in that category. For others the driver is environmental. The food, the stress, the sleep, the dopamine dysregulation. Those things can actually change. When that's the case, withdrawing the drug without touching the environment just returns you to the starting point. I'm not holding GLP-1 to a higher standard. I'm asking whether we're being honest about which situation we're actually in before we decide what long-term looks like.

“Eat less and move more” has been the advice for decades. Long-term success rate for severe obesity with diet and exercise alone: about 2–4%. If that worked reliably, we wouldn’t have spent 30 years developing GLP-1 therapies. And to the health coaches, low-carb influencers with supplement checkout lanes, and gym bros: the gym and good training absolutely matter. But shaming people with obesity and shouting “work harder” isn’t treatment. Stay in your lane. Medicine exists for a reason.

Nobody serious is arguing lifestyle doesn't matter. The real question is what happens when the drug stops. The STEP 1 trial showed two thirds of lost weight returns within a year of discontinuation. When the behavioral, inflammatory and hormonal environment that contributed to the condition remains unchanged, discontinuation data tells you exactly what the drug was doing. That's not a criticism of the medicine. It's a question about the goal. Are we treating obesity or are we treating the symptoms of an environment that remains unaddressed?
