Além da DLC "The Ballad of Gay Tony", focada na amizade de Luis e Gay Tony, o jogo também aborda diversos episódios de homofobia envolvendo o personagem Bernie Crane, levando Niko a protegê-lo de ataques homofóbicos a até eliminar um dos preconceituosos que ameaçou Bernie. E, mesmo sendo o típico personagem durão, Niko nunca diminui seus amigos por conta da sexualidade deles nem faz comentários maldosos.
